One of the main goals of the HDWD project was to increase the statewide epidemiologic capacity of health statistics on individuals with disabilities in Delaware. For that purpose, two surveys were developed and implemented in the community. Both surveys, the Adult Survey on Health, Wellness, and Lifestyle of People with Disabilities and the Childhood Survey on Health, Disabilities, and Quality of Life were based on the Centers for Disease Control's (CDC) Behavioral Risk Factor Surveillance System (BRFSS). The BRFSS is the largest telephone health survey system in the world and tracks health conditions and risk behaviors of the U.S. population.
Data from the surveys are currently being analyzed and reports on the findings of the surveys will be published early summer of 2007.
Adult Survey
The Adult Survey was conducted between January and November 2006 with adults 18 years of age and older that had physical and intellectual disabilities, mental health conditions, or visual and hearing impairments.
Participants were recruited through Delaware State registries, health fairs, grassroots organizations, and word-of-mouth.
The survey consisted of 134 questions and was completed in 45-60 minutes with trained interviewers at home, a public location or over the phone.
Each participant who completed the survey received a $15.00 check. A total of 339 Delaware adults with disabilities participated in the survey.

Childhood Survey
The Childhood Survey is currently being conducted and will continue until the end of May 2007. Parents and legal guardians of children with disabilities (physical, intellectual, sensory, genetic, etc.) and chronic health conditions (e.g. asthma, diabetes, cancer, etc.) age birth through 17 years can participate in the survey.
Participants are recruited through health fairs, camps, grassroots organizations, word-of-mouth, schools, and presentations to state and community organizations and agencies.
The survey consists of 60 general questions and additional questions divided by following age groups:
- Newborn to 5 years of age
- 6 to 11 years of age
- 12 to 17 years of age
Adolescents 12 years and older complete part of the childhood survey if they have received permission from their parents. Adolescents 14 years and older complete an additional Teen Risk Behavior Survey, also with permission from their parents.
All interviews are conducted by trained interviewers at the homes of the participating families or a public location of families' choice. The survey takes approximately 30 to 45 minutes. Each family receives a $15.00 check for their completion of the survey (one check per child).
